Transition Assessment
This service provides the expertise of a transition specialist to complete transition assessments (both informal and formal) and recommendations to the CSE to determine student's transition needs.
- Planning sessions with administrator/teachers
- Access to students to provide services
- Teacher/Staff participation in planning and programming
- Place to provide the services
- Access to class lists and IEPs for students we are working with
- Assistance in getting paperwork completed/copied to make applications to appropriate community agencies

- Communication between district and community organizations that relate to transition needs
- Updates on current trends/services related to transition
